License Renewal

Every active and inactive podiatrist will receive a renewal form by mail around the middle of May.  If you do not recieve one, you can print the following file and complete the information to renew your license to practice podiatry in the Commonwealth of Kentucky.  The file is available in Adobe Acrobat .pdf format.  If you are unable to print the file, call the Board office and we will mail you an application form.
