New License Application

Print the following files and complete the information to apply for a  license to practice podiatry in the Commonwealth of Kentucky.  The files are available in Microsoft Word (.doc format) or Adobe Acrobat (.pdf format.)  If you are unable to print the files, call the Board of Podiatry and we will mail you the forms.
